Are you accepting new patients?
Yes we are.
I’ve never worked with a dietitian before. How does this work?
Our dietitians provide individualized MNT, or medical nutrition therapy. We learn about you, your health concerns, your lifestyle,and your goals. You’ll meet periodically, as determined by you and your dietitian. In these meetings you’ll learn which actions to take to meet your goals. These actions can be about food, sleep, medications, exercise and other things that might be getting in the way of achieving your goals.
Do you take insurance?
We do. We’re in network with most major insurance plans, medicare, mediare advantage and even some smaller ones. As a courtesy, we’re happy to provide you with an estimate of coverage for our services, under your specific plan.
If insurance doesn’t cover, how much do you charge?
The price varies. From full sessions with a dietitian to meal planning only, to health coaching. We have plans for all budgets.
Can you check benefits before I schedule an appointment?
Absolutely!
Do you just tell me what to eat?
We certainly can but our most lasting results occur when we help our patients make changes in lifestyle habits that lead to lasting change and knowledge.
What does a nutrition program at Smart Plate include?
An initial consultation to learn about you. A game plan to get you where you want to be and a maintenance plan to help keep you there.
What is your area of expertise?
Weight Loss, Diabetes, Pre-diabetes, Women’s Health, High Blood Pressure, Gastrointestinal problems, Sports Performance, Fatty Liver, and General Nutrition.
Are meetings in-person or virtual?
We provide our services in-person, virtually through a secure video platform, and telephone
Do I need a doctor’s referral to get scheduled?
If you have a medicare, medicare advantage, HMO or POS plan, you will need a referral. Some POS plans do not. Check with your plan administrator.
Is this a one time thing? Or do I have to come back for follow ups?
While most of our patients work with us over time to improve health and reach their goals, there are occasions where 1 meeting is what fits in the budget. We understand and will tailor our services to meet patient needs.
Do you prescribe pills or shots?
We do not but can work with your physician if needed.
Do you do any testing?
Yes, we use body composition testing, genetic testing and micronutrient testing to help inform our patients’ Care Plans.
